Description
Clinical Manager - / Home Health
- Position: Home Health RN – Clinical/Documentation; in-office position, 40 hours/week, Monday–Friday.
- Schedule: 8 hours/day | 40 hours/week | Total contract weeks: 35.
- Clinical Responsibilities: Review and process OASIS, 485s, evaluation packets, orders, medication profiles, wound/vital-sign alerts, and ensure accurate HCHB documentation.
- Case Management: Lead weekly case conferences, review recertification/discharge decisions, coordinate continued/add-on services, and communicate with physicians for required orders.
- Referral & Compliance: Manage referral intake, ensure timely assessment visits, review documentation for accuracy/compliance, and address deficiencies or non-admits in HomeCare HomeBase (HCHB).
- Additional Duties: Monitor missed visits, authorizations, billing-related documentation, labs, and on-call reports; may provide PRN patient visits/on-call coverage or function as a staff nurse when needed.
Required Qualifications
- Active RN license in the state of employment
- Minimum 2 years nursing experience, including 1 year Home Health
- 1+ year OASIS experience
- 1+ year HomeCare HomeBase (HCHB) experience
- Valid driver's license and automobile liability insurance
- Strong documentation, communication, organization, and computer skills
- Knowledge of federal/state/local home health regulations
