Recent Home Care experience within the last 12 months
Active Arizona RN license
No hits/issues on state license
BLS from AHA, ARC, or ASHI
Valid driver's license for residency verification
Banner Cover Page completed
Home Care skills checklist completed within the last 12 months
Resume must include:
- All facilities worked
- Departments
- Trauma level
- Number of beds
RTO ≤ 7 days
No RTO during the first week
Must be ready to accept the assignment at submission

Screening Summary Best candidate profile: RN with 2+ years Home Care experience, including recent experience within the past year + active AZ RN license + BLS + valid driver's license + travel experience preferred. ⚠️ Critical: If the candidate is an Arizona resident, they are not eligible for this Arizona assignment.
