RTO Restrictions: No more than 7 days
Guaranteed Hours: 3 shifts per 13 weeks. The cancelled shifts must be the regular shift length to which the Contractor Personnel is assigned

Special Requirements:
These patients are end of life (prognosis of 6months or less) so they come to us with a variety of dx. Our top dx we most frequently see are Heart Failure, Heart disease, COPD, & Cancers but we have other such as Parkinson's, Alzheimer's, etc.
Metro Hospice area could include MO and/or KS patient caseload so ideally candidate is licensed in both KS and MO. MO is most preferred if they are not dual licensed. Counties served are: MO: Jackson, and parts of Clay, Cass, and in KS Johnson and Wyandotte.

This is a home hospice position so they would either help manage a caseload (14-16pts) or they would help other case managers offload their routine visits.

Visits are completed on patients 1-2 times per week and typically last around 1 hour each.

For Full-time need-hour are Monday - Friday - 8am-5pm (There is flexibility if they want in their start and end times, that is just fine).
As this is a position in Home Hospice they must have reliable transportation.
