Shift Matrix: Evening Shift | 15:00 – 23:30 (5x8-Hour Shifts)

• Work Schedule: 40 guaranteed hours per week (5x8-Hour Shifts, 15:00 – 23:30). Every other weekend and every other holiday rotation required.

• On-Call Requirement: No call.

🚫 Absolute Minimum Qualifications & Departmental Hardstops:

• Licensure Mandate: Active, unencumbered New York State RN License held in hand at submission.
• Experience Baseline: Minimum 1+ year of recent Skilled Nursing, Long-Term Care, or Post-Acute RN experience in an acute/sub-acute setting.
• Mandatory Certifications: Active AHA BLS.
• Flexibility & Role Adaptability Hardstop: Candidate MUST agree to float to other units as needed and drop down to an LPN Medication Pass Cart role when staffing levels dictate.
• Orientation Schedule: Orientation is strictly limited to 1 day on assigned unit; candidate must be ready to work autonomously immediately following orientation.

Job Summary & Clinical Scope

A Skilled Nursing Facility in Montour Falls, NY is seeking an experienced Registered Nurse Team Lead (RN SNF) for a 13-week evening contract in SNF 3 (Department 3005). Operating within a 120-bed skilled nursing facility (3 units, 40 residents per unit), the RN Team Lead will oversee evening nursing operations, lead unit staff, direct resident care plans, and provide direct clinical support.

• Clinical Team Leadership & Supervision: Function as the Professional RN Team Lead on a 40-resident unit, managing shift operations and supervising unit LPNs and CNAs (Standard Shift Ratio: 1 RN, 2 LPNs, 3–4 CNAs per 40 residents).
• Resident Assessment & Delegation: Direct ongoing physical assessments, evaluate acute changes in condition, delegate nursing duties within state scope of practice, and lead interdisciplinary care planning.
• Medication Administration & Cart Coverage: Execute high-volume medication administration, wound care treatments, and enteral feedings; step in directly to run LPN medication carts as operational staffing dictates.
• Documentation & Emergency Response: Document complete resident progress notes and care interventions in the EMR, manage physician orders, and direct acute medical interventions or emergency room transfers.
