Requirements:
 

-Education: BSN required; Master's degree in Nursing, Health Administration, or Business Administration preferred.
 

- License: Currently active state license required.
 

- Experience: Minimum 5 years of experience in an acute-care healthcare setting.
 

- Leadership: Minimum 5 years of progressively responsible leadership or administrative experience in Case Management, Care Coordination, Utilization Management, or a closely related function.
 

- Certifications: BLS/BCLS required; CCM or ACM certification preferred.
 

- Leadership Skills: Demonstrated experience leading interdisciplinary clinical and operational programs, managing staff, and driving performance improvement.
 

- Clinical Knowledge: Advanced knowledge of case management, care coordination, utilization management, discharge planning, patient progression, and transitions of care in an acute-care setting.
 

- Regulatory Compliance: Experience with regulatory requirements and accreditation readiness.

EMR: EPIC experience preferred.

Shift: Day shift, Monday–Friday
 

Critical Requirement: Must have previous leadership experience in Care Coordination.
