Position Overview

Seeking a Physical Therapist (PT) with Home Health experience to provide physical therapy services to patients in the home setting. The PT will evaluate patients, develop treatment plans, provide direct therapy, and monitor progress toward functional goals.

Key Responsibilities

• Evaluate patients' functional abilities, including ROM, strength, coordination, mobility, and independence.
• Review and implement physician referrals.
• Establish patient functional baselines and treatment goals.
• Develop individualized treatment plans and estimate treatment duration.
• Provide direct physical therapy treatments according to the established plan.
• Educate patients, families, and the community on appropriate therapy techniques and care.
• Participate in interdisciplinary discussions and patient-care planning.
• Document evaluations, treatment plans, treatments, and patient progress in the electronic health record.
• Modify treatment plans as needed based on patient progress.
• Participate in quality improvement and department programs.
• Provide education and in-services in areas of expertise.
• Cross-train or provide coverage within Rehabilitation Services when needed.

Required Qualifications

• Home Health experience – REQUIRED
• Minimum 1 year of experience
• Active Michigan PT license – REQUIRED
• Current BLS (AHA) – REQUIRED
• Valid Michigan driver's license in good standing
• Reliable vehicle available for daily business use
• Proof of auto insurance
• Ability to travel throughout the assigned coverage area

• Infants
• Pediatrics
• Adolescents
• Adults
• Geriatrics

Assignment Details

• 5×8-hour day shifts
• 8:00 AM – 5:00 PM
• 40 hours/week
• No call
• Weekend rotation every 4–6 weeks
• Approximately 6 visits/day
• Patient ratio listed as 5
• Weekend requirement: approximately 3 weekend shifts per contract

Compliance Requirements

• RTO: Maximum 10 days
• Labs must be scheduled within 48–72 hours of contract signing
• Proof of flu vaccination required at submission
• Medical exemption may be requested within 48 hours of signing
• Supervisor-level references required
• Travelers are first to float if needed
• Background/license hits must be disclosed during submission with a personal statement and require facility approval
• Previous Bronson internal travelers may return after 6 months
• Previous permanent Bronson staff may return after 1 year
• Modules: 8 hours, non-billable, completed during orientation and annually

Important Note

Home Health experience is required. Candidates should also be prepared for daily travel within Van Buren, Calhoun, and Kalamazoo Counties and must have a vehicle, valid driver's license, and insurance.
