5x8 08:00-16:30 Monday-Friday
2 years of experience required
- acute rehabilitation, discharge planning, interdisciplinary collaboration, and care coordination are required
- Acute Hospital experience required
Must have completed at least 1 travel assignment
BLS required
LMSW, LCSW, and MD license required in hand
This facility requires that all employees have the Flu vaccine*
Travel experience required:
- Yes
Note: If candidate has relevant/current hospital Case Management experience on med/surg unit. Certfications:Certs marked with * are preferred
LCSW, LCSW-C, LMSW, MD LMSW/LCSW License
