Description

Pride Health is actively hiring a Physical Therapist to join our client's Home Health Team.

 

Location: Winter Haven, FL 33880
Patient Care Setting: Home Health
Schedule: Full-Time; Regular Day Shift
Travel: Local Travel Required

Position Overview:

  • We are seeking a compassionate and skilled Physical Therapist (PT) to provide high-quality rehabilitative care to patients in their homes.
  • The Physical Therapist will assess patient needs, develop and implement individualized treatment plans, and collaborate with physicians and interdisciplinary team members to restore, maintain, and improve patient mobility, independence, and overall well-being.

Key Responsibilities:

  • Perform comprehensive physical therapy evaluations and ongoing assessments to determine patient needs, functional limitations, and appropriateness for therapy services.
  • Develop, implement, document, and revise individualized Plans of Care (POC) in collaboration with physicians and interdisciplinary care team members.
  • Provide therapeutic interventions for patients with neuromuscular, neurological, orthopedic, and other medical conditions based on physician referrals and clinical evaluations.
  • Assess patient mobility, balance, strength, coordination, endurance, and safety needs within the home environment.
  • Educate patients and caregivers on therapeutic exercises, transfer techniques, fall prevention strategies, and home safety practices.
  • Train patients and families on the safe use of assistive devices including walkers, wheelchairs, canes, braces, prosthetics, and other adaptive equipment.
  • Evaluate home environments and recommend appropriate durable medical equipment (DME) and environmental modifications to maximize safety and independence.
  • Monitor patient progress, document treatment outcomes, and communicate changes in condition to physicians and interdisciplinary team members.
  • Supervise and collaborate with therapy assistants and other healthcare professionals as appropriate.
  • Maintain accurate and timely clinical documentation, including evaluations, progress notes, care plans, physician communications, and discharge summaries.
  • Apply infection control and universal precaution practices during all patient care activities.
  • Participate in team meetings, case conferences, and quality improvement activities.

Required Qualifications:

  • Current State License as a Physical Therapist (PT).
  • Bachelor's Degree in Physical Therapy from an American Physical Therapy Association (APTA) approved program.
  • Current CPR Certification.
  • Current driver's license, reliable transportation, and proof of automobile insurance.
  • Minimum 1-2 years of experience as a Physical Therapist in an acute care, rehabilitation, or related clinical setting.
