LPN/LVN – Home Health
Pride Health is hiring an LPN/LVN – Home Health to provide skilled nursing care to patients in their homes under the supervision of an RN Case Manager.
Job Details
- Setting: Home Health / Hospice
- Location: Little Rock, AR
- Job Type: Contract
- Schedule: As assigned; possible on-call duties
- Travel: Local travel required
Key Responsibilities
- Provide skilled nursing care under the direction of the RN and established Plan of Care (POC).
- Assess patient needs, monitor outcomes, and report changes to the RN Case Manager.
- Assist with admissions, discharges, and patient transfers.
- Educate patients and families on self-care, safety, and disease management.
- Maintain accurate clinical documentation using HomeCare Homebase (HCHB).
- Coordinate with patients, families, physicians, RNs, and interdisciplinary teams.
- Assist with specialized care for Home Health/Hospice patients as needed.
- Follow infection control, Medicare, state, and federal requirements.
Requirements
- Education: Graduate of an accredited LPN/LVN or Practical/Vocational Nursing program
- License: Current LPN/LVN license in the state of practice
- Certification: Current CPR certification
- Experience: 2 years of LPN/LVN clinical experience preferred, with Home Health or Hospice experience strongly preferred
- Technology: HomeCare Homebase (HCHB) and electronic clinical documentation
- Other: Valid driver's license, reliable transportation, current auto liability insurance, and required TB/Hepatitis documentation
