Job Details:
Schedule: Monday-Friday | 8:30 AM-5:00 PM | 40 Guaranteed Hours/Week | On-Call/Weekend: Every Other Weekend
Location: Jacksonville, FL; Coverage areas include Nassau and Duval Counties; Mileage reimbursed between visits, from first patient to last patient.
Job Type: Contract

*Pay offered is based on experience, expertise, credentialing, and education.

Duties:
• Initiate, develop, implement, and revise individualized plans of care in collaboration with physicians and healthcare professionals while ensuring clinical competence and quality patient care.
• Supervise and coordinate care provided by licensed practical vocational nurses, provide instruction, assign tasks according to State and federal regulations, and complete required supervisory visits.
• Complete accurate and timely OASIS assessments, medication reconciliation, nursing evaluations, patient documentation, and home health eligibility and medical necessity requirements.
• Communicate effectively with physicians, patients, families, agency staff, schedulers, and other clinicians to coordinate visits, orders, patient care needs, schedule changes, and quality care.

Requirements:
• License(s): Active RN license
• Certification(s): BLS
• Years of experience: 1+
• Skills/Specialties/Technology: HCHB/Pointcare proficient; OASIS experience required
