If youāre used to running from patient to patient all day, this assignment is something different. This Transitional Care OT opportunity in North Hollywood offers a lower-volume caseload designed around more individualized, meaningful patient care.

- MondayāFriday | 8:00 AMā4:30 PM
- 13-week contract with extension available
- Adult population 55+
- Work with patients managing multiple chronic conditions
- Mix of clinic-based care, home-safety visits & post-hospital discharge care
- Approximately 50/50 coverage between North Hollywood & Pasadena
- Administrative time and responsibilities built into the role
- Reliable transportation required
