Description

Travel LMSW – Social Services

Facility: Providence Swedish Medical Center – Edmonds Campus
 

Schedule

  • Shift: Days, 5x8 hours- Hours: 8:00 AM – 4:30 PM
  • Primarily Monday–Friday, with some weekend coverage required
  • Must be willing to work 1 of 3 holidays: Thanksgiving, Christmas, or New Year's
  • Float: Yes — very unlikely to float to other Swedish hospitals, but travelers may be required to float to nearby sister facilities

Requirements

  • Minimum 2 years of experience
  • Acute Care / Medical-Surgical hospital experience is required
  • Hospital experience required
  • Case Management / Utilization Review experience
  • Valid LMSW or MSW
  • BLS
  • Epic experience required

Key Responsibilities & Skills

  • Case management and care coordination
  • Discharge planning
  • Biopsychosocial assessments
  • Initial evaluations and development of plans of care
  • Medical necessity reviews
  • Continued stay and concurrent reviews
  • Appeals and denials
  • Coordination of home care, equipment, rehabilitation, and long-term care services
  • Patient and caregiver education
  • Care conferences and interdisciplinary collaboration
  • Referrals to community resources and support services
  • Knowledge of Medicaid, Medicare, medical transportation, and medication assistance
  • Knowledge of protective services, guardianship, advance directives, and power of attorney
  • Documentation and communication related to patient care and discharge planning
  • Experience with adult and geriatric patients; pediatric/infant exposure may also be encountered
