Description
Pride Health is actively hiring a RN Case Manager - Hospice to join our client's Hospice Team.
Location: Mesa, AZ 85204
Patient Care Setting: Hospice
Weekly Hour Guarantee: 40 Hours
Coverage Area: Up to a 60-mile radius from the agency
Schedule: Monday-Friday, 8:00 AM - 4:30 PM
Productivity Expectation: 30 Points
On-Call Requirement: Yes, rotating every other week (4:30 PM - 8:00 AM Monday-Friday)
Weekend Requirement: Possible rotating weekend coverage
Mileage Reimbursement: Yes, reimbursed between patient visits
Dress Code: Clean, professional scrubs of any color; free from wrinkles, tears, holes, fraying, or odors
Position Overview
We are seeking an experienced and compassionate Hospice Registered Nurse (RN) Case Manager to provide exceptional end-of-life care to patients and families throughout the Mesa service area. The RN Case Manager will manage an assigned hospice caseload, coordinate interdisciplinary care, provide symptom management, and support patients and families through all phases of the hospice journey.
Key Responsibilities
- Manage a hospice caseload of up to 15 patients while ensuring high-quality, patient-centered care.
- Perform comprehensive hospice assessments and develop individualized plans of care.
- Complete all visit types including routine visits, admissions, assessments, recertifications, symptom management visits, and end-of-life care visits.
- Respond to patient needs, changes in condition, and urgent clinical situations in a timely manner.
- Provide compassionate pain and symptom management interventions to maximize patient comfort and quality of life.
- Conduct hospice admissions and coordinate initiation of services.
- Perform death visits and post-mortem responsibilities in accordance with agency policies and hospice regulations.
- Educate patients, caregivers, and family members regarding disease progression, medications, symptom management, and hospice services.
- Coordinate care with physicians, social workers, chaplains, hospice aides, pharmacists, and other interdisciplinary team members.
- Maintain accurate, timely, and compliant documentation within Homecare Homebase (HCHB).
- Participate in rotating after-hours and weekend on-call schedules as assigned.
Required Qualifications
- Active Arizona Registered Nurse (RN) License or Compact RN License.
- Recent Hospice Nursing Experience.
- Recent Homecare Homebase (HCHB) experience.
- Strong case management and hospice care coordination skills.
- Current driver's license, reliable transportation, and proof of automobile insurance.
- Ability to travel throughout the assigned service area.
