Description

We are seeking an experienced Registered Nurse Case Manager to provide comprehensive, patient-centered care in an ambulatory/primary care setting in Alaska.

The RN Case Manager will utilize the nursing process to assess, plan, implement, and evaluate patient care while coordinating services across the healthcare continuum. This role includes patient education, medication and treatment administration, care coordination, referrals, follow-up, and collaboration with providers and interdisciplinary teams.

The ideal candidate will have strong case management experience, clinical judgment, care coordination, and patient advocacy skills and be comfortable working in a fast-paced outpatient environment.

Required Qualifications

  • Active, unrestricted Alaska RN License – REQUIRED at submission
  • Graduate of an accredited School of Nursing.
  • Recent Case Management experience in a clinic or hospital setting within the last 2 years – REQUIRED
  • BLS certification – Required
  • High school diploma or equivalent.
  • Strong clinical assessment and care coordination skills.
  • Ability to work effectively in an ambulatory/primary care environment.
