Description
Pride Health is actively hiring a Registered Nurse (RN) to join our client's Hospice Team.
Work Address: Fort Smith, AR 72903
Patient Care Setting: Hospice
Weekly Hour Guarantee: 32 Hours
Schedule: Monday-Friday, 8:00 AM - 5:00 PM
On-Call Requirement: Backup On-Call Rotation
Weekend Requirement: Possible Rotational Coverage as Needed
Coverage Area: Northwest Arkansas, including Sebastian, Logan, Crawford, Franklin, Scott, and Yell Counties.
Mileage Reimbursement: Yes, reimbursed between patient visits
Dress Code: Grey, Navy Blue, or Black Scrubs
Position Overview
- We are seeking a compassionate and dedicated Hospice Registered Nurse (RN) to provide high-quality, patient-centered hospice care throughout Northwest Arkansas.
- The RN will be responsible for delivering skilled nursing services, coordinating patient care, supporting families through the end-of-life journey, and collaborating with the interdisciplinary hospice team to ensure comfort, dignity, and quality of life for patients.
Key Responsibilities
- Provide skilled nursing care to hospice patients in home and community-based settings.
- Perform comprehensive patient assessments and ongoing evaluations to identify physical, emotional, psychosocial, and spiritual needs.
- Develop, implement, and update individualized plans of care in collaboration with physicians and interdisciplinary team members.
- Manage pain and symptom control while ensuring patient comfort and quality of life.
- Monitor patient conditions and communicate significant changes to physicians and the care team.
- Educate patients, caregivers, and family members regarding disease progression, medications, symptom management, and hospice services.
- Coordinate care with physicians, social workers, chaplains, hospice aides, and other healthcare professionals.
- Provide emotional support and guidance to patients and families throughout the hospice journey.
- Maintain accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
- Participate in backup on-call rotations and provide clinical support as assigned.
Required Qualifications
- Active Registered Nurse (RN) License in Arkansas or a qualifying Compact State.
- Current and unrestricted RN licensure.
- Strong clinical assessment and patient care skills.
- Valid driver's license, reliable transportation, and proof of automobile insurance.
- Ability to travel throughout the assigned service area.
- Ability to work independently while providing compassionate end-of-life care.
