• Miles: Approximately 125 miles/week

Territory

The RN will provide home health services in:
Fitchburg, Leominster, Lancaster, Lunenburg, Princeton, Westminster, Sterling, Clinton, Holden, Boylston, and West Boylston.

Responsibilities

• Provide skilled nursing care through home visits.
• Complete Start of Care (SOC), wound care, follow-up visits, and other adult home health services.
• Manage approximately 5–10 patient visits per day, based on patient needs and territory.
• Complete accurate and timely documentation using HCHB.
• Perform OASIS assessments as required.
• Attend periodic office meetings.
• Coordinate care with physicians, patients, families, and other healthcare professionals.
• Monitor patient conditions and report changes appropriately.
• Provide patient and family education regarding medications, treatments, and care plans.

Requirements

• Minimum 1 year of recent adult Home Health experience.
• First-time travelers are not accepted.
• Current CPR and BLS certification.
• Active RN license required; pending licenses are not accepted.
• Must have recent HCHB experience and be proficient in the system. HCHB must have been used within the past 6 months; retraining may be required if experience is older.
• OASIS proficiency required.
• Reliable transportation, valid driver’s license, and required liability insurance.
• Must be willing to travel within the assigned territory.

Additional Information

• Locals accepted at the same rate.
• Mileage between patient homes is reimbursable at the IRS standard rate.
• Travel to the first patient and from the last patient is not billable.
• Dress code: Scrubs.
• No guaranteed hours; if patient census is low, the clinician may contact the Account Manager regarding additional hours at a nearby clinic.
