Job Requirements:

• Doctorate/Bachelor’s/Master’s Degree in Physical Therapy from an accredited program
• Minimum 6 months of Outpatient Physical Therapy experience
• Active California Physical Therapist License
• CPR Certification
• Must maintain professional licensure in good standing
• Must comply with California State Practice Act and applicable regulatory requirements
• Strong clinical reasoning and patient-care skills
• Experience developing and modifying individualized treatment plans
• Ability to complete accurate and timely medical documentation
• Must be comfortable working in an outpatient setting
• No New Graduates

Job Responsibilities:

• Perform comprehensive evaluations of assigned patients.
• Develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction.
• Provide physical therapy treatments focused on functional recovery and return to community activities.
• Monitor patient progress and establish appropriate discharge goals.
• Complete timely and accurate documentation, including evaluations, treatment notes, goals, functional assessments, medical-necessity documentation, and charge entry.
• Ensure treatment is provided under valid physician orders and within applicable insurance authorization.
• Educate patients, families, students, and coworkers regarding treatment and rehabilitation.
• Maintain patient and employee safety, including safe patient handling and infection-control practices.
• Participate in student clinical education when assigned.
• Maintain continuing education and professional development.
• Participate in quality-improvement initiatives and follow organizational policies and procedures.
