Description

Key Responsibilities:

Conduct biopsychosocial assessments and assist with treatment planning.
Provide individual/family therapy and case management.
Coordinate discharge planning, aftercare, and community resources.
Collaborate with physicians, NPs, and clinical staff.
Support risk assessments and care coordination.

Requirements:

Master’s Degree in Clinical Social Work required.
LCSW or LMHC license in Massachusetts required.
