Facility: Atlantic Health System, Morristown Medical Center, 100 Madison Ave, Morristown, NJ 07960
Department: Care Management (Morristown)
Start Date: 11/02/2026 | End Date: 01/30/2027 (about 13 weeks) | Positions: 3
Pay: $62.00/hrs
Schedule: Days, 8:00 AM to 4:00 PM, 5x7.5 hrs (37.5 hrs/week)
Weekends/Holidays: Rotating weekends (every 4th or 5th); one summer and one winter holiday
Role Summary: Coordinates patients through acute care hospitalization, manages discharge planning and transitions, and performs utilization management and quality screenings. In-patient case management experience is required.
Must-Haves
- Active RN license
- BSN (required)
- 2+ years of acute inpatient hospital discharge planning
- In-patient case management experience
- Experience with managed care
- Experience placing patients in rehabilitation facilities
- Experience with subacute patients
- English read, write, and speak
Preferred: CCMC certification, 2+ years of acute care experience
