📌 Position

• Role: LPN/LVN
• Setting: LTC / Skilled Nursing Facility
• Contract: 13 weeks
• Experience: 1+ year required
• Travel experience: Not required
• Locals: Not accepted
  • Candidates living within 70 miles of the facility are considered local.

🕐 Shift & Schedule

• Shift: Mid shift
• Hours: 2:00 PM – 11:00 PM
• Schedule: 5 × 8-hour shifts
• Weekends: Every other weekend at minimum
• Holidays: As required
• Float: Yes — two floors / two wings per floor
• Call: Not required

👩‍⚕️ Required

• LPN/LVN
• 1 year experience
• BLS
• LTC / Skilled Nursing experience
• PointClickCare experience preferred

• Foley catheter insertion — male & female
• Insulin administration
• IV therapy / starting and maintaining IVs
• Blood glucose monitoring
• Neurological/stroke assessment
• Dressing changes
• Skin/wound assessment
• Wound VAC
• JP drains
• Colostomy/ileostomy care
• Feeding tube care
• Tracheostomy care
• CPAP/BiPAP
• Oxygen administration
• Medication reconciliation
• Pain assessment/management
• Isolation precautions/PPE
• Hoyer lift
• Diabetes monitoring
• End-of-life/palliative care

• Patients with acute medical care needs
• Daily census: 75
• 24 beds listed
• 12 rooms listed

👥 Ratios

• Days: 1:24
• Nights: 1:48
• Weekends: 1:24

⚠️ These are very high ratios, so make sure the candidate understands the workload.

💻 Charting

• PointClickCare
• Experience is preferred.

👕 Dress Code

• Any solid-color scrubs

💉 Vaccination

• COVID vaccine required
• Medical/religious exemptions are stated as allowed for the COVID requirement.

🛡️ Compliance – VERY IMPORTANT

OSHA Respirator Clearance:

• Required before start for subcontractor agency travelers.
• Agency cost: $22

Fit Testing:

• Traveler must shave to allow the respirator mask to seal.
• Failure to shave for fit testing can result in cancellation.
• Facility does not provide CAPR/PAPR.
• Facility does not allow travelers to use their own CAPR/PAPR.
• The posting says no medical or religious exemptions for this respirator requirement.

⏳ Traveler Time Limit

• Clinicians cannot work as travelers at the facility/system for more than 1 year.
• After 1 year, a 6-month break is required before returning as a traveler.

• Allied modules: approximately 1 hour
• Modules are recorded on transcripts.
