Description

REQUIRED EXPERIENCE

Minimum 2 years of pharmacy experience
Strong inpatient/acute care hospital experience required — hard stop
Must have acute care experience within the last 3 years
Do not submit candidates whose only recent experience (within the last 3 years) is in retail pharmacy
Epic experience required
Experience with medication reconciliation, discharge medication management, patient counseling, and post-discharge follow-up
Experience making medication recommendations and implementing medication changes with a high degree of autonomy
Experience with interdisciplinary care rounds and collaboration with hospitalists
IV medication experience, pharmacotherapy management, and therapeutic drug monitoring
Experience with antibiotics, anticoagulation, renal dosing, cardiac dosing, and high-alert medications
Pyxis and Carousel System experience preferred
Ability to work independently with strong communication skills
First-time travelers accepted

SCHEDULE

Day shift: 7:00 AM–3:30 PM
Mid shift: 2:30 PM–11:00 PM
Monday–Friday

REQUIRED CERTIFICATIONS

PharmD or B.S. in Pharmacy required
RPh preferred
Pharmacy residency from an accredited ASHP institution preferred

LICENSE

Active Virginia pharmacist license required
Must be in good professional standing in Virginia
