SCHEDULE

• Shift: Day Shift, 5×8 hours
• Hours: 8:00 AM – 5:00 PM
• Schedule: Monday–Friday
• Weekends: No weekends
• Contract Duration: 13 weeks (confirm)
• Case Load: Approximately 10 new patients per week
• Patient Population: Adults
• Work Setting: Primarily outpatient, with inpatient evaluations as needed
• Holiday Requirement: Must work 3 of 5 holidays — Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
• Float: May be required to float to nearby sister facilities
• Call: Not specified

RESPONSIBILITIES

• Complete psychosocial evaluations for pre- and post-liver transplant patients.
• Conduct initial biopsychosocial assessments.
• Verify patient caregivers over the phone.
• Provide inpatient evaluations as needed.
• Assess psychiatric disorders and drug/alcohol use and treatment options.
• Assist with advance directives and Power of Attorney documentation.
• Provide crisis intervention when required.
• Educate patients and caregivers on individualized care plans and treatment options.
• Document assessments, care plans, goals, and patient/caregiver education.
• Participate in care conferences and interdisciplinary collaboration.
• Coordinate social services, interpretation services, and transportation resources.

REQUIRED DOCUMENTS

• Updated resume with complete work history.
• MSW degree documentation.
• LICSW license documentation, as required.
• Valid required state license/compact eligibility — must be confirmed and in hand at submission per posting.
• Two supervisory/professional references covering at least one year of work within the last three years; charge RNs/leads may be accepted.
• All RTO and scheduling requests disclosed at submission.
• Providence employment history confirmation: Candidate must disclose any previous permanent, travel, registry, or per-diem employment at a Providence facility, including location and dates.
• Tier 3 travel compliance documentation.
• SSN and DOB for submission/compliance through the authorized process
