Job Summary

We are seeking a compassionate and dedicated Registered Nurse (RN) to provide quality nursing care to patients in a rehabilitation and long-term care (LTC) setting. The RN will be responsible for assessing patient conditions, administering medications, coordinating care, and supporting patients in their recovery and daily healthcare needs.

Key Responsibilities

• Assess patients' health conditions and monitor vital signs.
• Administer medications and treatments as prescribed.
• Develop and implement nursing care plans in collaboration with the healthcare team.
• Provide wound care and assist with other nursing procedures as needed.
• Monitor patients' progress and report changes in condition to the appropriate healthcare provider.
• Maintain accurate and timely patient records and nursing documentation.
• Collaborate with physicians, therapists, nursing assistants, and other healthcare professionals.
• Educate patients and families regarding treatment plans and ongoing care.
• Follow infection control protocols, facility policies, and patient safety standards.

Requirements

• Active and valid Registered Nurse (RN) license for the state of practice.
• Previous nursing experience in rehabilitation, long-term care, or skilled nursing facilities preferred.
• Strong clinical assessment, communication, and documentation skills.
• Ability to provide safe, compassionate, patient-centered care.
• Current BLS certification, as required by the facility.
