Position: Medical Social Worker / Discharge Planner

Facility: Providence-affiliated acute care hospital

Shift: Days, 5 × 8 hours

Schedule: 8:00 AM – 4:30 PM

Patient Population: Adults and geriatric patients

Caseload: Up to 25 patients per day

Contract Duration: 91 weeks listed; confirm the actual contract length with the recruiter.

Floating: No routine floating listed in the job description; however, floating between nearby sister facilities may be required under the assignment terms.

Required Qualifications

• Minimum 2 years of acute medical social work experience, including both social work and discharge planning.
• Education/Credentials: MSW or LCSW
• Charting System: Epic experience required.
• Travel Experience: Required, although first-time travelers may be considered subject to screening.
• License: Required state license; compact license must be in hand at submission, per posting.
• References: Two professional references covering at least one year within the last three years.

Key Responsibilities

• Conduct patient and caregiver needs assessments.
• Coordinate care and develop individualized plans of care.
• Manage discharge planning and arrange durable medical equipment (DME).
• Participate in interdisciplinary care conferences and family meetings.
• Support patients experiencing homelessness, substance use disorders, mental health crises, and other complex social needs.
• Assist with advance directives, POLST documentation, and protective services processes.
