Description
Physical Therapist (PT) | Mokena/Frankfort, IL
Shift: 8-hour days; covers the Mokena/Frankfort territory.
Requirements:
- Minimum 1 year of Home Health PT experience required
- Active Illinois PT license and BLS
- Valid driver's license and car insurance
- COVID-19 vaccination required; declinations not accepted
- OASIS and Start of Care experience strongly preferred
Responsibilities: Patient assessments, individualized treatment plans, mobility rehabilitation, functional independence, and timely EMR documentation for pediatric through geriatric patients.
