Description
Physical Therapist (PT) – Home Health | Baytown, TX
- Schedule: Monday–Friday, 8:00 AM–5:00 PM; no weekends/holidays unless approved in advance
- Hours: 40 GT per week
- Contract: 8–13 weeks
- Region: Houston
- On-Call: None
- Charting: WellSky
- Mileage & Drive Time: Both paid at applicable rates; drive time considered in scheduling.
- Productivity: 32–34 points/week; 1 point per visit
- Start of Care: Not expected; if added, likely 3 points due to AI
- Certifications: BLS and First Aid
- Experience: Minimum 2 years of recent licensed PT experience, including home health experience. No new graduates. First travel assignment accepted with home health experience.
- Housing: Candidates must research housing before submission.
- Reporting: Must call in all missed visits, evaluation frequency issues, and unsuccessful patient contact attempts.
- Dress Code: Clean, professional scrubs/attire and closed-toe shoes; small nose stud and earrings permitted.
