📍 Location: Saginaw, Michigan

🏥 Setting: Skilled Nursing Facility / Rehabilitation

Monday-Friday
9:00 AM - 5:00 PM
One Saturday per month
40 hours per week

Overview

🦿 Provide physical therapy services to residents in a skilled nursing and rehabilitation setting.

📋 Responsibilities include:

Patient evaluations
Individualized treatment plans
Therapeutic interventions
Care coordination
Progress monitoring
Patient and family education
Documentation within NetHealth

85-90% productivity expectation

📚 One-day facility orientation followed by on-the-job training
