Job Description: Registered Nurse (RN) – Hospice

Facility: Confluence Health Hospital – Central Campus
Location: Wenatchee, Washington

Shift Details

• Shift: Day/Evening (D/E)
• Schedule: 5 shifts per week, 8 hours per shift
• Hours: 40 hours per week
• Shift Timing: 8:00 AM – 4:30 PM

Required Qualifications & Certifications

• Active Washington State or Compact RN license.
• Current Basic Life Support (BLS) certification.
• Must comply with Confluence Health policies, unit-specific standards of practice, and the Washington Nurse Practice Act.
• Preferred Experience: Epic EMR, particularly Dorothy/Comfort Care.

Key Responsibilities

• Coordinate and provide high-quality, compassionate hospice nursing care using the nursing process and established standards of practice.
• Develop, implement, and continually reassess individualized patient care plans.
• Administer medications and perform necessary nursing procedures and treatments.
• Conduct ongoing, holistic patient assessments and respond appropriately to changes in condition.
• Accurately document patient assessments, care plans, interventions, and responses.
• Collect relevant patient information to ensure safe, effective, and individualized care.
• Evaluate discharge plans, identify educational needs, and make necessary adjustments.
• Address patients’ physical, psychological, social, and rehabilitative needs while promoting comfort and dignity.
• Educate patients, families, and caregivers regarding health conditions, care plans, and appropriate care practices.
• Maintain current knowledge of clinical workflows, processes, and evidence-based nursing practices.
• Delegate appropriately and oversee care provided by non-licensed assistive personnel.
• Collaborate with physicians and interdisciplinary healthcare team members to ensure continuity of care.
• Participate in performance evaluations and quality improvement initiatives with the Director or Clinical Manager.
• Maintain a clean and safe environment in accordance with hospital and unit policies.
