Job Description: 

Licensed Practical Nurse (LPN)

Facility: CareOne at Essex Park
Location: 265 Essex Street, Beverly, MA 01915

Contract & Shift Details

• Contract Duration: 4 weeks
• Scheduled Hours: 40 hours per week
• Shift: 5 shifts per week, 8 hours per shift
• Available Shifts: Evening (3:00 PM – 11:00 PM) and Night (11:00 PM – 7:00 AM)
• Flexibility: Must be willing to work both evening and night shifts as needed to fill scheduling gaps.
• Cancellation Policy: The facility may cancel one shift per week without penalty.

Key Responsibilities

• Coordinate and oversee the nursing process for assigned residents.
• Provide direct and indirect nursing care in accordance with each resident’s individualized nursing and medical care plan.
• Ensure care is delivered in compliance with state and federal regulations and accepted nursing standards.
• Supervise Certified Nursing Assistants (CNAs) and licensed nursing staff within the assigned discipline, as applicable.
• Ensure staff follow established clinical procedures, regulatory requirements, and professional practice guidelines.
• Monitor residents’ conditions and communicate changes to the appropriate healthcare team members.
• Maintain accurate resident records and clinical documentation.
• Support resident safety, comfort, and quality of care.

Required Qualifications

• Graduate of an accredited nursing program.
• Active Massachusetts LPN license or applicable license recognized for practice in Massachusetts.
• Current Basic Life Support (BLS) certification.
• Proficiency with PointClickCare (PCC) EMR.
• Completion of Massachusetts-required dementia training: initial 8 hours, followed by 4 hours annually.
