Mandatory Requirements

• Minimum 12 months of permanent Med/Surg Telemetry experience.
• Must have a permanent Ohio RN license or compact license in hand at submission.
• BLS (AHA) and ACLS — active and in hand at submission.
• One manager, ANM, supervisor, or director reference in the specialty, from an assignment worked within the past 12 months.
• No coworker references accepted.
• No license hits accepted.

Required Certifications

• BLS
• ACLS
• NIHSS is listed in the qualifications section; confirm whether it is mandatory for this specific unit.

Submission Requirements

• Completed Standard Qualivis Cover Sheet.
• Requested time off (RTO) must be discussed during the interview and included at submission.
• Candidates may be submitted for only one job at a time within the Cleveland Clinic Health System.
• Background hits are reviewed, but candidates with an open case are not accepted.
• Nurses are temporarily exempt from the nicotine-free facility requirement.

Orientation and Compliance

• BCI/FBI fingerprinting must be completed within 48 hours of signing the contract.
• ECG rhythm recognition and SafeMedicate assessments are completed during orientation.
• Candidates get a maximum of two attempts to pass both assessments; failure may result in cancellation.
• PBDS assessment is waived.
• Approximately 6–15 hours of pre-start modules; only 4 hours are billable.
• Permanent and per diem employees must wait one year after their previous end date before returning to Cleveland Clinic hospitals.

Vaccination Requirements

• COVID-19 vaccine required; medical or religious exemptions accepted.
• Flu vaccine required; medical or religious exemptions accepted.
