Description
Job Title: Licensed Practical Nurse (LPN)
Facility Type/Setting: Skilled Nursing Facility
Shift & Schedule: 3:00 PM – 11:30 PM; 3 days per week Weekly Hours: 24 hours
- Active LPN license
- Experience with PointClickCare preferred
- BLS
Responsibilities:
- Assist with patient assessment, care planning, implementation, and evaluation.
- Administer medications and treatments under RN supervision and facility policy.
- Monitor vital signs and report changes in patient condition.
- Provide person-centered care and advocate for patients.
- Collaborate with the interdisciplinary care team and keep the supervising RN informed of changes.
- Maintain clinical competency and complete required education.
- Document patient care accurately in the EMR, preferably PointClickCare.
