Coverage area: Oakdale, Maplewood, Woodbury May be asked to help in other territories within facility's service area as needed which is about a 60 mile radius from the Facility address LVNs are required to get 32 points per week, routine visits .9.
Must have recent Home health exp 2025-26
HomeCare HomeBase experience required
Client may consider without HCHB exp at a reduced rate for the first 2 weeks if the manager is willing to train.
