- Work with older adults, generally ages 55 and up, to support independence and quality of life
- Help patients improve their ability to perform daily activities and manage everyday challenges
- Develop individualized treatment plans based on each patient's functional needs and goals
- Collaborate with an interdisciplinary care team and involve caregivers when appropriate
- Enjoy a more personalized approach to care, with a smaller patient caseload than many traditional settings
