Position: Registered Nurse (RN) – Home Health
Location: Illinois
Shift: Days, Monday–Friday
Schedule: 8:00 AM–5:30 PM / 8:00 AM–5:00 PM
Guaranteed Hours: 40 hours per week
Contract Duration: 91 weeks (confirm with the account manager)
Pay Rate: $80 (confirm rate basis)
On-Call: No
Weekends: Not required

Job Summary

The Home Health RN is responsible for assessing patients, developing and implementing individualized plans of care, coordinating services, and providing skilled nursing care in patients’ homes. The nurse monitors patient progress, evaluates care needs, coordinates discharge planning, and ensures compliance with federal, state, and agency regulations.

Key Responsibilities

• Conduct comprehensive initial and follow-up assessments of patients’ physical, psychosocial, and environmental needs.
• Develop, implement, and modify individualized plans of care as needed.
• Complete OASIS assessments and Start of Care (SOC) documentation.
• Coordinate care with physicians, patients, family members, caregivers, and other healthcare professionals.
• Assess patient and caregiver education needs and provide appropriate teaching.
• Supervise CNAs and coordinate assigned healthcare staff.
• Complete documentation according to regulatory and agency guidelines.
• Maintain patient privacy, confidentiality, and a safe care environment.

Required Qualifications

• Previous Home Health experience — Required
• Active BLS certification or willingness to obtain it — Required
• Valid driver's license and car insurance — Required
• Active Illinois nursing license; a copy must be included with submission — Required
• COVID-19 and flu vaccination — Required; declinations will not be approved
• Strong Med-Surg experience with some Home Health experience may be considered.

Preferred Experience

• OASIS documentation
• Start of Care (SOC) visits
• Home Health patient assessments and care planning
