Required Qualifications:

Doctor of Pharmacy (PharmD) or equivalent from an accredited pharmacy college
2-year ASHP residency in HIV/HCV or qualifying infectious disease specialization
5 years of direct HIV patient care experience involving medication management
Texas pharmacy license or eligibility for Texas licensure
BLS certification
Required HIV/HCV-related board certification and specialized residency credentials
