Physical Therapist – Outpatient

Job Description:
 

NovaCare Rehabilitation is seeking an experienced Outpatient Physical Therapist to evaluate patients, develop individualized treatment plans, deliver therapeutic interventions, and support patients in achieving their functional goals and returning to community activities.

The Physical Therapist will provide high-quality, patient-centered care while maintaining accurate documentation, adhering to physician orders, and following applicable state and federal regulations. The role also involves collaboration with patients, families, coworkers, and students to promote safe and effective rehabilitation services.
 

Key Responsibilities:

• Perform comprehensive patient evaluations and develop individualized treatment plans based on patient needs, functional goals, and physician direction.
• Implement and modify treatment plans to support patient progress and timely discharge.
• Provide outpatient physical therapy evaluations and treatments, including 60-minute sessions.
• Maintain clear, accurate, and timely medical records, including progress notes, goals, functional assessments, letters of medical necessity, and charge entry.
• Ensure treatment is provided under valid physician orders and within applicable insurance authorizations.
• Educate patients and families on treatment plans, functional improvement, and safe practices.
• Participate in student supervision and education when assigned.
• Maintain professional standards, ethical conduct, and compliance with state practice regulations.
• Participate in professional development and clinic quality-improvement initiatives.
• Follow patient safety, infection control, safe patient handling, and workplace safety procedures.

Required Qualifications:

• Degree in Physical Therapy from an accredited school.
• Minimum 1 year of outpatient clinical experience.
• Active and applicable Ohio Physical Therapist license.
• Current CPR certification.
• Must not be a new graduate.
