RN Team Lead – Skilled Nursing Facility (SNF)
Location: Montour Falls, NY 14865

Assignment: 11/01/2026 – 01/30/2027
Duration: 13 weeks
Shift: Evenings, 3:00 PM – 11:30 PM Schedule: 5 evenings/week, 40 hours/week Openings: 1 Experience: 1 year required On-Call: No Orientation: Typically 1 day on assigned unit Scrubs: Any color
Critical Submission Guardrails
- License: Verify active New York State RN license.
- Experience: Confirm relevant RN/SNF experience; 1 year is listed as required.
- Schedule: Every other weekend and every other holiday required.
- Floating: Must be willing to float to other units as staffing permits.
- Role Flexibility: May be required to step into an LPN medication-cart role when needed and staffing permits.
- Physical Requirements: May need to lift up to 50 lbs. and stand or sit for extended periods.
- Status: Replacement opening due to employee resignation/termination.
Job Summary & Clinical Scope
Seeking an RN Team Lead for a 120-bed skilled nursing facility with three units, each serving up to 40 residents.
- Lead nursing activities through effective assessment, clinical judgment, prioritization, delegation, and time management.
- Monitor residents’ conditions, document findings, and promptly report changes in health status.
- Coordinate care with interdisciplinary teams and advocate for residents’ needs.
- Maintain accurate EMR documentation and follow ethical, professional, and evidence-based nursing practices.
- Support medication-cart duties when needed and staffing permits, and participate in unit meetings and staff councils.
Unit & Staffing Details
- Facility Capacity: 120 beds across 3 units, with up to 40 residents per unit.
- Staffing Ratio: 1 RN, 2 LPNs, and 4–5 CNAs per 40 residents on the evening shift.
- Schedule: 5 evening shifts per week; every other weekend and holiday.
- Floating: Other units as staffing permits.
