Travel Physical Therapist (PT) – Home Health
Sarasota, Florida
We are seeking a Travel Physical Therapist for a 20-week Home Health contract in Sarasota, Florida, covering the Sarasota/Charlotte County area. This opportunity is a great fit for an experienced home health PT who enjoys independent field-based care and helping patients improve mobility, strength, safety, and functional independence in their homes.
Local candidates are accepted at the same rate.

Responsibilities
- Perform comprehensive home health PT evaluations and reassessments.
- Develop individualized treatment plans based on patient needs and functional goals.
- Provide gait, balance, transfer, strengthening, mobility, and therapeutic exercise interventions.
- Assess patients for walkers, wheelchairs, transfer equipment, and other assistive devices.
- Treat orthopedic, neurological, cardiopulmonary, post-surgical, and medically complex patients.
- Educate patients and caregivers on home exercise programs, medications, safety, and injury prevention.
- Complete timely home health documentation and coordinate care with the interdisciplinary team.
- Identify and report changes in patient status and signs of potential abuse or safety concerns.
Requirements
- Florida PT license required.
- BLS required.
- Home Health experience required.
- Minimum 1 year of experience required.
- OASIS training preferred.
- Valid driver’s license and reliable vehicle required for home visits.
- Strong documentation and independent case-management skills required.
