Physical Therapist – Outpatient Rehab | Travel Contract

Shift: Day Shift – 4x10s (6:45 AM–5:00 PM)
Schedule: Flexible 8-hour or 10-hour shifts | No Weekends

Position Highlights:

• Minimum 1 year of PT experience required
• BLS certification required
• Cerner charting system
• First-time travelers welcome
• Local candidates accepted — no mileage restriction
• 25-bed Critical Access Hospital
• Free parking with a facility-issued permit
• Time-off requests and block scheduling considered by the manager

Key Responsibilities:

• Perform patient assessments and physical therapy evaluations
• Develop and implement individualized treatment plans
• Provide therapeutic exercises, mobility training, and gait training
• Assist patients with rehabilitation, strength, range of motion, and functional mobility
• Document evaluations, progress, reassessments, goals, and discharge plans
• Educate patients and families on home exercise programs, safety, and injury prevention
