Travel Medical Social Worker (MSW) | Mission Viejo, CA | $2,761.00/week

Location: Mission Viejo, California
Contract Length: 13 Weeks
Shift: Days | 8:00 AM – 4:30 PM
Schedule: 5x8-hour shifts
Caseload: Approximately 20–23 patients
Charting System: Epic

Job Requirements:

• Minimum 1 year of acute-care inpatient hospital experience required; recent hospital experience within the last 3 months is specified in the requirements.
• Previous travel experience is mandatory. First-time travelers will not be accepted.
• Epic experience is a strict requirement.
• MSW required; LCSW preferred.
• BLS required.
• Acute-care medical social work, case management, and discharge planning experience required.
• Behavioral health-only or Emergency Department-only social work experience will not be accepted.
• One RTO request allowed, with no more than 7 days off requested.
• Days off may vary and are not guaranteed to be consecutive.
• Manage discharge planning and care coordination for adult and geriatric inpatients.
• Complete initial patient assessments within 24 hours.
• Develop individualized plans of care and coordinate post-acute services.

• Coordinate referrals to rehabilitation, long-term care, and other appropriate facilities.
