Travel Contract – Intake Coordinator - Clinical

• Schedule: Monday–Friday, 8:00 AM–5:00 PM (5x8-hour shifts)
• Pay: $1,812/week ($45.30/hour)

Job Description

The Intake Coordinator – Clinical processes home care referrals and coordinates with clinical staff, referral sources, and community liaisons to ensure patients are entered into the system, scheduled for visits, and that all stakeholders receive timely status updates.

This role also provides clinical guidance, identifies educational opportunities, and evaluates patients’ conditions, progress, and ongoing needs to support case managers in understanding and addressing home care requirements.

Requirements

• Current RN, LVN, or LPN license in the state of practice.
• CPR certification.
• At least 1.5 years of relevant experience.
• Epic experience required.
• Healthcare experience in a clinic, home care, skilled nursing facility (SNF), or hospital setting preferred.

Education & Experience

• Associate degree or technical degree in Nursing, or an equivalent combination of education and related experience — required.
• Three years of home care experience — preferred.
