Physical Therapist – Home Health | Travel | Portland, OR
Location: Portland, OR
Assignment length: 13 weeks
Start date: 11/22/2026
Schedule: Day shift, 8:00 AM – 4:30 PM
Work week: 5 x 8-hour shifts
Setting: Home Health
About the Assignment
We’re seeking a Home Health Physical Therapist for a 13-week assignment in Portland, OR. You’ll be assigned your own territory and visit patients in their homes. Paid orientation begins on a Monday at the start of the assignment, and modules are completed during orientation. On-call rotation may be required and will be discussed at interview. Travel pairs are welcome, though each traveler covers a separate territory with their own transportation. Timely communication and documentation are essential in this role.
Requirements
- Active OR PT license (must be in hand at submission)
- Minimum 1 year of PT experience. First-time travelers are not eligible.
- Home Health experience with HCHB (Homecare Homebase)
- OASIS experience required for start-of-care visits
- Valid driver’s license and reliable personal transportation
- Completed Hospice/Home Health checklist
