We’re looking for a Physical Therapist (PT) to support residents in a skilled nursing facility (SNF) setting in Granbury, Texas. This opportunity is ideal for a PT who enjoys helping patients regain strength, improve mobility, and build confidence as they work toward greater independence.

You can expect to:

• Evaluate residents and develop individualized plans of care based on their needs and functional goals.
• Provide therapeutic interventions to improve strength, balance, endurance, and mobility.
• Assist residents with gait training, transfers, and functional movement to promote safety and independence.
• Monitor patient progress and adjust treatment approaches as appropriate.
• Educate residents, families, and caregivers on exercises, mobility techniques, and fall prevention.
• Collaborate with the interdisciplinary healthcare team to support quality care and positive patient outcomes.
• Complete documentation in accordance with facility guidelines and professional standards.
