Description
Schedule
- Shift: Day shift
- Days: Monday–Friday
- Timing: 7:00 AM–3:00 PM
- Hours: 40 hours/week guaranteed
Required Experience
- Acute care experience — Required
- Licensed Master Social Worker (LMSW) — Required
- State licensure as a Licensed Bachelor Social Worker (LBSW) — Required
Required Certifications
- BLS — Preferred (AHA, American Red Cross, or accepted equivalent)
Job Responsibilities
- Assess patients' psychosocial, financial, placement, and community service needs.
- Identify barriers to discharge and develop appropriate discharge plans.
- Coordinate care with patients, families, physicians, RN Care Managers, and other healthcare professionals.
- Arrange community resources, appointments, and post-acute services.
- Assess readmission risks and support safe transitions of care.
- Participate in family care conferences and interdisciplinary meetings.
- Document assessments, interventions, discharge plans, and barriers in the EMR.
- Advocate for patients and families in complex cases.
