Referral Bonus

• Facility: Avita Health System, Ohio
• Shift: Nights, 7 PM–7 AM, 3 shifts per week
• Contract: 13 weeks
• Start date: November 16, 2026
• License: Active Ohio or compact RN license; pending licenses are not accepted
• Experience: Minimum 2 years of recent, continuous experience
• Certifications: BLS, ACLS, PALS
• Charting: Epic preferred
• Floating: Must be willing to work at all three campuses — Ontario, Bucyrus, and Galion
• Local candidates: Accepted at the same bill rate

Important submission requirements

• Two recent supervisory references, placed immediately after work history.
• Include all requested time off (RTO) with submission.
• No time off permitted for major holidays.
• Candidates with prior terminations, contract cancellations, or employment-history red flags should not be submitted.
• Review the Avita Traveler Expectations Document with the candidate and document their agreement.
• Up to 8 hours of required pre-start modules through HealthStream; non-billable.
• COVID-19 and flu vaccines required; medical or religious exemptions only.
• Smoke-free facility and campus.
• If placed on call, the traveler must be able to arrive at the hospital within one hour of being called in.
