 Job Description – RN Case Manager

Position: Case Manager – Acute Care
Facility: TriStar Centennial Medical Center
Location: 2300 Patterson St., Nashville, TN 37203
Department: Case Management
Cost Center: 924
Shift: 8:00 AM – 4:30 PM
Schedule: Weekdays and weekend days
Guaranteed Hours: 40 hours/week
Contract Length: 13 weeks
Facility Bed Count: Approximately 450 beds
Patient Ratio: 1:30–1:35
Documentation Systems: Midas, Meditech, and NaviHealth
 

1. Required Experience

• Previous travel experience in acute care case management is required.
• Must have experience using Midas and Meditech.
• Must be able to adapt quickly to the facility's workflow.
• Expected to work productively after 5 days of orientation.
• Must be comfortable managing a high caseload.

2. Required Skills

• Acute care case management.
• Midas documentation system.
• Meditech for patient reviews.
• NaviHealth utilization.
• Patient review and case management workflows.
• Ability to manage approximately 30–35 patients.
• Ability to work across multiple units as needed.

3. Schedule Requirements

• Shift: Day shift, 8:00 AM–4:30 PM.
• Schedule: Weekdays and weekend days.
• Guaranteed Hours:  40 hours per week.
• Floating: May be required to work across more than one unit depending on staffing needs.

5. Dress Code
