Job Description – Nurse Case Manager

Facility: Kittitas Valley Healthcare
Location: Ellensburg, Washington
Program: WHSN Travel – Clinical
Contract Duration: 13 weeks stated; 90 weeks listed in the system — confirmation required
Start Date: ASAP

1. Position Details

Shift: Day shift, 8:00 AM–4:30 PM
Schedule: 5 days per week, 8-hour schedule
Guaranteed Hours: 40 hours per week
Facility Type: Critical Access Hospital
Facility Size: 25 beds
EHR System: Cerner

2. Required Qualifications

Minimum 1 year of experience in case management, social work, and discharge planning, as stated in the posting.
Proficiency with the Cerner Electronic Health Record (EHR) system.
