Registered Nurse (RN) – Acute Care Case Manager

Facilities: Petaluma Valley Hospital and Healdsburg Hospital
 

Shift and Schedule

• Day shift: 8:00 AM – 4:30 PM
• 5 days per week, 8-hour shifts.
• Every other weekend required.
• Must float between both facilities, approximately 30 minutes apart.
• Weekly schedule may vary between locations depending on staffing and PTO.

Required Experience and Skills

• Minimum 2 years of experience listed in job qualifications.
• At least 1 year of acute-care hospital Case Management experience is a hard requirement.
• Epic experience required — no exceptions.
• InterQual criteria experience required.
• Active Compact RN license must be in hand at submission.
• Travel experience required.
• Experience with care coordination, admission criteria, discharge planning, and CMS guidelines.

Patient and Unit Details

• Adult and geriatric patients.
• Daily census: approximately 30–35.
• Patient ratio listed as 1:15.
• Interdisciplinary support includes PT, respiratory therapy, radiology, social services, pharmacy, transportation, and unit secretary.
