Job Description:
We are seeking an experienced Registered Nurse (RN) Case Manager to coordinate and optimize patient care across the continuum, from acute care through discharge and post‑acute services. The RN Case Manager will collaborate with physicians, nursing staff, social work, utilization review, and external providers to ensure safe, timely, and cost‑effective transitions while maintaining high quality and regulatory compliance.

Key Responsibilities

·        Conduct comprehensive clinical assessments and care planning for patients across medical, surgical, and chronic‑care populations.

·        Develop, implement, and monitor individualized care plans in collaboration with the interdisciplinary team and patient/family.

·        Coordinate discharge planning, including home health, skilled nursing, rehab, durable medical equipment, and community resources.

·        Facilitate timely authorizations and approvals with insurance payers and managed care organizations.

·        Monitor length of stay, avoidable delays, and readmission risk using data tools (e.g., LACE+, risk scores, or internal metrics).

·        Collaborate with social work, financial counseling, and patient navigation teams to address social determinants of health and barriers to care.

·        Utilize EHR and case‑management software to document assessments, interventions, and outcomes.

·        Participate in quality improvement initiatives and provide education to patients, families, and staff on care coordination and discharge processes.

Requirements

·        Active RN CA license.

·        Minimum 2–3 years of acute‑care nursing experience (hospital, observation unit, or similar setting).

·        Experience in case management, utilization review, discharge planning, or care coordination preferred.

·        Strong understanding of Medicare/Medicaid, managed care guidelines, and payer requirements.

·        Proficiency with EHR systems and electronic documentation.

·        Excellent communication, negotiation, and problem‑solving skills.
