Weekends: Every other weekend required; every weekend preferred.

Job Requirements:
- Valid California license required at the time of submission.
- Minimum 2 years of experience in case management.
- Acute hospital experience.
- Experience with care coordination, discharge planning, disease management, and needs assessments.
- Knowledge of DME ordering and plan-of-care development.
- Familiarity with MS, SDU/PCU/IMC/Observation units, and surgical services.
- Experience with Allscripts charting system required.
